Information about the Pain Diary
The pain diary is designed to help track how well your pain is managed. To make the best use of the diary, please record your pain level 3 times per day. Also, indicate how the pain is affecting your daily activities, if at all. Make comments regarding any side effects you feel may be caused by your pain medicine, when they occur, and what you do about these side effects.
The goal in providing this diary is to help people work with their physicians to gain maximum pain relief and the highest level of function with minimal side effects.

a. You will be able to track you pain through out the day and what medications you needed to take in order to help your situation.
b. In the comment section you can document if the medication taken was for a severe spike in pain or if it was the normal pain medication taken throughout the day.
Prescription Tab (click image to enlarge)

a. You will be able to track all of your prescriptions. It will list your last refill so you can track when your prescription is running out.
b. You will make notes on why you were prescribed the medication.
c. There will be a history kept of Active and In-Active prescriptions.

a. This is a list of preset Pain Locations such as Back, Neck, etc.
b. Looking at the top of the Screen Shot you will notice you can add your own pain location. This will help you be as specific as you wish.
Pain Type (click image to enlarge)

a. This is a list of preset Pain Types that is linked to the Pain Location.
b. You can also add Pain Types that are specific to the individual.
